Concussion Information
This is the sheet that the student athlete and a parent or guardian must sign in agreement to allow for partcipcation in interschoastic athletics.
Travel with another parent/Transport another student
Athletic & Activity Release Guidelines to be used when a student is to be transported by someone other than their parent or guardian. Arrangements must be approved by building administration in advance of the trip. Adult picking up the student must be added as an emergency contact in Skyward It is the parent or guardian’s responsibility to enter the emergency contact in Skyward prior to the trip. Proof of emergency contact info must be attached to this sheet. Adult transporting the student will be asked to provide identification upon pickup.
